Access to coaching and coaching resources, is given only for the term of coaching agreement, and will not be continued outside of the 8 week term, unless otherwise specified or additional services are purchased.
- Waiver
Prior to being provided with Coaching Services, The Client must complete a health screening form, and advise the coach of any and all known health conditions, and medical history.
By purchasing the Eat to Honour Program, the client acknowledges that the coach is not a registered dietician, and that any dietary nutritional advice received is to be considered general guidance only, and is not to be considered professional advice. The client understands that the coach is not a psychologist, or therapist, and any coaching advice received is to be considered general guidance only, and is not to be considered professional advice. The coach will not be providing psychoanalysis, behavioural therapy or psychological counselling. Coaching may include setting priorities, strategising and organisation, asking clarifying questions and providing examples and models.
The Coach and the business is hereby released for any and all injuries, illnesses, ailments, damages, and loss of income that I may suffer as a result of the Client’s nutrition, training and lifestyle choices.
In accepting these Terms of Service, the Client declares that they have been cleared by my medical practitioner to safely pursue nutrition and lifestyle coaching, with Empowered Evolution, and have been cleared of any pathologies or medical conditions that could nullify or render this agreement void.
This is the entire agreement between yourself and the Company; it supersedes all other negotiations and discussions prior to the execution of this agreement. You agree that no other representations have been made by the Company to induce you into entering into this agreement and no modification to the terms of this agreement shall be effective
unless in writing signed by both parties.
